When you enrolled in LIFE-NWPA you received an identification card that looks like this. Please, be sure to keep this card on you at all times. It is very important to present this identification card at all doctor’s appointments and when you require emergency room services.

How to Appoint a Representative

According to Medicare guidelines, an “Appointed Representative” is a person who can act on your behalf to request and exception, appeal or grievance. A representative can be a friend, relative, or caregiver that you trust to help you make decisions and to help you figure things out with regards to your healthcare.
